Process, Tools, and What to Expect at Jacobs Clinic
Attention Deficit Hyperactivity Disorder (ADHD) is often first identified in childhood, but it can persist into adulthood and may go undiagnosed for many years. Without diagnosis and appropriate support, ADHD can affect education, employment, relationships, and overall wellbeing.
Jacobs Clinic provides specialist ADHD assessments for both adults and children, using structured, clinically recognised tools to support accurate diagnosis and tailored treatment recommendations.
What Is ADHD?
Attention Deficit Hyperactivity Disorder (ADHD) is a neurodevelopmental condition that affects a person’s ability to manage attention, control impulses, and regulate activity levels. It can persist into adulthood and may be underdiagnosed, particularly in individuals who do not display symptoms typically associated with childhood ADHD.
There are three recognised presentations of ADHD:
Predominantly Inattentive Type
Predominantly Hyperactive-Impulsive Type
Combined Type (involving both sets of symptoms)
Adult ADHD Symptoms
ADHD in adults may differ in presentation from ADHD in children. Common symptoms include:
Difficulty focusing or sustaining attention
Disorganisation and poor time management
Forgetfulness
Impulsivity
Restlessness or an inability to relax
Challenges with completing tasks or following through on responsibilities
Secondary issues, such as low self-esteem, anxiety or mood difficulties, may also be present and can be linked to unmanaged ADHD symptoms.
ADHD Assessment at Jacobs Clinic
Jacobs Clinic offers comprehensive ADHD assessments for both adults and children.
The assessment process is structured and evidence-based:
1. Registration and Questionnaires
Patients complete a registration form and a series of questionnaires. These cover ADHD symptoms, medical history, and general mental health. A third-party questionnaire is also included, to be completed by parents, partner or friend. For children and young persons, a couple of questionnaires are sent to the school or college contact.
2. QbCheck or QbTest
This computer-based task recognised by NICE and the NHS is completed at home (QbCheck) or in the clinic (QbTest). It measures attention, impulsivity and activity. The results are compared to a large normative database by age and sex.
3. Clinical Interview
Patients undertake a full mental health assessment with a specialist clinician. For adults this may take place in person or online, depending on preference and clinical suitability. For children and young persons, initial assessment is always in person at Jacobs Clinic.
4. Results and Care Plan
If a diagnosis of ADHD is confirmed, the clinician will provide a full report, explain the findings, and outline treatment recommendations.
What Are QbTest and QbCheck?
QbTest and QbCheck are objective tools developed by QbTech and recognised by NICE. They support the diagnosis of ADHD by measuring core symptoms during a computer-based task.

QbTest (Clinic-Based)
Conducted in the clinic using a motion-tracking camera and specialised equipment. At Jacobs Clinic this test is used for children and young persons as part of their ADHD assessment.
QbCheck (Remote)
Completed remotely at home using a standard computer and webcam. At Jacobs Clinic this is used for adult patients.
Both tools are FDA approved, CE marked, and recognised for their reliability in supporting clinical assessments.
After the Assessment
Following diagnosis, patients receive:
A full written diagnostic report
Personalised recommendations for treatment or support
Signposting to relevant therapies, coaching and initiating medication.
The report is suitable for sharing with other healthcare professionals involved in ongoing care.
Shared Care With Your GP
Jacobs Clinic encourages shared care arrangements. Patients are advised to speak with their GP before the assessment to discuss shared care options. With consent, the clinic will liaise with the GP to obtain medical information before the assessment. We will also request that the GP take on prescribing under a shared care agreement.
Once a patient is stabilised on medication, shared care enables the GP to manage prescriptions. 90% of the patients show significant improvement in their symptoms within a month of starting ADHD medication. Most of our patients are transferred to shared care with their GP once they are stable on medication. It takes around 3 months from initiation of treatment to shared care in the majority of patients.
